In regard to the Central La. Community Theatre dba City Park Players’ production of the show stated above, which is scheduled to be performed on the Hearn Stage on the dates listed above, I hereby agree to make every effort to attend ALL scheduled rehearsals, and will commit my presence to all technical and performance dates AND to participate to the best of my ability in the strike of the set. I grant to Central La. Community Theatre dba City Park Players, the right to take photographs of me, and my family, in connection with the above-identified event. I authorize Central La. Community Theatre dba City Park Players, its assigns and transferees to copyright, use, and publish the same in print and/or electronically. I agree that Central La. Community Theatre dba City Park Players may use such photographs of me with or without my name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and Web content. I have read and understand the above.
